General Information
The patient is always responsible for the prompt payment of his/her bill regardless of unsettled insurance disputes, or pending reimbursement from insurance companies. Einhorn, Mandas and Bradley Rehab Associates, Inc. will submit all insurance forms as a service to the patient.
Please make sure the we have all of your correct insurance information on file. Charges are based on services rendered and are never contingent upon the outcome of a lawsuit, disability judgment or other dispute.
We are not a participating provider for all insurance plans. Each individual insurance plan provides different amounts of physical therapy coverage. Please contact your insurance company to verify your physical therapy benefits and to review your policy for coverage and reimbursement for treatment. If other billing or insurance questions arise, contact our office for assistance.
Workers' Compensation
Our facility handles all types of Workers' Compensation Claims. If your injury is covered by Workers' Compensation, please provide us with your company's name, address and contact person so that our bill can be processed swiftly. If Workers' Compensation refuses to honor your claim, please remember that you are responsible for our payment.
Medicare
Both of our offices have received Medicare certification. Billing will be handled by our staff as a convenience for the patient. Contact our office for additional information.
